Does Remote EMDR Work?

Yes. Multiple studies published between 2021 and 2024 consistently show that remote EMDR produces symptom reduction comparable to in-person delivery. The research is clear enough that EMDRIA (the EMDR International Association) supports telehealth EMDR with appropriate preparation and safety protocols.

Key findings from the research:

  • Therapeutic alliance ratings remain stable across delivery methods
  • PTSD symptom reduction is comparable to in-person treatment
  • Clinicians report equivalent confidence in remote vs. in-person outcomes
  • Remote EMDR expands access to trauma treatment for rural and underserved populations

Technical Setup Checklist

Before your first remote EMDR session, verify each item.

Your Setup (Therapist)

  • Stable internet connection (wired ethernet preferred over WiFi)
  • External monitor or large screen (the BLS tool works best at larger sizes)
  • Headset with microphone (reduces echo, improves audio quality)
  • Screen sharing enabled in your telehealth platform (Zoom, Doxy, Google Meet)
  • BLS tool bookmarked and tested: coralehr.com/tools/emdr-bilateral-stimulation
  • Client session link ready (use "Go Live" for real-time sync)

Client Setup

  • Private, quiet space with a door that closes
  • Stable internet connection
  • Screen large enough to track visual BLS (tablet or laptop minimum, phone is too small)
  • Headphones for auditory BLS (bilateral tones require stereo separation)
  • Emergency contact information shared with therapist
  • Grounding objects within reach (for closure/containment if needed)

Two Methods for Delivering BLS Remotely

Method 1: Screen Sharing

The simplest approach. Open the BLS tool, go fullscreen, and share your screen through your telehealth platform. The client sees the moving dot on their screen exactly as you see it on yours. You control speed, pattern, and atmosphere from your side.

Pros: zero client setup, works with any telehealth platform. Cons: slight video compression delay (typically 50-200ms), no audio BLS through screen share.

Click "Share with client" in the BLS tool. This auto-starts a live session and copies a link you send to your client. They open it on their device and see the BLS animation in real-time, controlled by you. No screen sharing needed. The connection uses WebSocket for near-zero latency.

Pros: no video compression, client gets full-quality animation, audio BLS works through client's headphones. Cons: client needs to open a link (one extra step).

Safety Protocols for Remote EMDR

Remote EMDR requires more preparation than in-person. You can't physically intervene if a client becomes highly activated.

  • Digital safety plan — verified emergency contacts, local crisis numbers, explicit permission to contact support persons
  • Grounding protocol — establish a remote safe-place/container exercise before processing begins
  • Technology disruption plan — what happens if the connection drops mid-processing? Agree on a protocol (client calls you, uses grounding exercise until reconnected)
  • Assessment of client readiness — evaluate whether the client can self-regulate if the session is disrupted. Not all clients are appropriate for remote EMDR.
  • Titrated approach — consider starting with less activating targets remotely before processing core trauma

Saving Per-Client Configurations

Each client has preferences: some track better at speed 3, others at speed 7. Some prefer the calm atmosphere, others want clinical white. Save a template for each client so you can load their exact configuration in seconds at the start of each session.

In the BLS tool, configure the settings, click "Save Template," name it (e.g., "Alex — Calm 5x"), and it's saved. Next session, open your templates and load it with one click.
