Track SUD and VOC During EMDR Sessions
This free SUD/VOC tracker is designed for trained EMDR clinicians who want a lightweight way to record baseline, between-set, installation, closure, and reevaluation ratings. Keep entries de-identified, copy the summary, then paste it into your clinical note or EHR.
Example SUD/VOC tracking pattern
A clean session summary should show ratings over time, not just a final score.
| Checkpoint | SUD | VOC | Visual trend |
|---|---|---|---|
| Baseline | 8 | 2 | |
| Set 1 | 6 | 3 | |
| Set 2 | 4 | 4 | |
| Set 3 | 3 | 5 | |
| Install | 2 | 6 | |
| Closure | 1 | 6 | |
What Are SUD and VOC in EMDR?
SUD, often written SUDS, stands for Subjective Units of Disturbance. In EMDR sessions, it is commonly rated from 0 to 10 to capture present-moment distress while focusing on target material. VOC stands for Validity of Cognition and is commonly rated from 1 to 7 to capture how true the positive cognition feels.
How to Use the Tracker During BLS Sets
- 1. Enter the target setup - Add a de-identified target label, negative cognition, and positive cognition.
- 2. Capture baseline - Record the starting SUD and VOC before processing begins.
- 3. Add rows between sets - After each BLS set, record SUD, VOC, client report, and body sensation.
- 4. Mark closure status - Note whether the target was complete, incomplete, contained, or needs follow-up.
- 5. Export the summary - Copy or download a clean text summary for your documentation workflow.
Therapist Guide to SUD and VOC Tracking
For a deeper walkthrough of when to capture baseline ratings, between-set changes, installation VOC, closure status, and reevaluation data, read the SUD and VOC tracking guide for EMDR therapists. It includes examples for documenting incomplete targets without overclaiming clinical resolution.
Documentation Example
Therapist used EMDR SUD/VOC tracking during processing of a de-identified target. Client reported baseline SUD 8/10 and VOC 2/7. Across BLS sets, client reported reduced disturbance to SUD 3/10 with shifting body sensation. Target remained incomplete at closure; therapist completed containment and grounding, with reevaluation planned next session.
